The following forms should be printed and forwarded to the office if your child is a new participant:  New Families Registration Form, Financial Agreement, Policies & Procedures Signature Page, Consent for Mutual Exchange of Information, Notice of Privacy, Teacher Observation Checklist, Informed Consent Form

The following forms should be printed and forwarded to the office if your child is continuing with us:  Continuing Families Registration Form, Financial Agreement, Consent for Mutual Exchange of Information (if your child has a new therapist, school, counselor, etc), Teacher Observation Checklist (if your child has a new teacher at school)
